*Please be aware that your registered domicile is included in your driver's license or passport, or submit it in a way that conceals your registered domicile.

A power of attorney proving that the agent is acting on behalf of the individual (requires the principal's handwritten signature and seal)
Documents for verifying the agent: A copy of one of the following: driver's license*, passport*, or health insurance card
*Please be aware that your registered domicile is included in your driver's license or passport, or submit it in a way that conceals your
registered domicile. Note 1. The "Request Form for Disclosure of Personal Information," the documents required for identity verification, and the documents required for agent verification that you submit when making a request will only be used for the purpose of disclosure and will not be used for any other purpose.
